Enrolment form

If you have studied the information provided and wish to enrol your child (or yourself) for classes you can do so by completing parts 1 and 2 of the enrolment form and return this, together with the enrolment fee (not required for Pay As You Go classes), to: 28 Prykes Drive, CHELMSFORD, Essex CM1 1TP.

The enrolment form is included within the information pack or can be downloaded by clicking on the links below. There is a waiting list for many classes and you will be notified when space becomes available (space is allocated on a first-come first-served basis amongst those who have submitted a completed form and enrolment fee).
